**Overview** St. Luke's is looking for a **Case Manager (RN)** to join us in our strategy to be the community's trusted partner in providing exceptional, patient- centered care in Nampa, Idaho. **Job Requirements:** + Works as clinician, consultant, educator and researcher to facilitate collaborative management of patient care. + Evaluates patient progress toward outcomes by monitoring the goals, care plan and revising plan of care as necessary. + Determines need for therapeutic medical, psycho-social and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives and plans. **Required Criteria:** + Bachelors' of Nursing Degree + 2 years' of RN experience + Current RN License in the State of Practice.
