Position overview Position title: Case Manager - Care Cord - BSW Job ID: 567864 Status: Full time Regular/Temporary: Regular Hours: Shift: Facility: UPMC Altoona Department: Case Management Location: 620 Howard Avenue, Altoona PA 16601 Description UPMC Altoona - Case Manager-Care Coordinator - Fulltime - 11am-11:30pm - six 12 hr shifts and one 8 hr shift per pay period - Every other weekend Purpose: Provide psychosocial assessments for any/all patients. Assume responsibility for discharge planning activities for all inpatients, and for any outpatients with an identified need. Responsibilities: * Expertise to coordinate and facilitate resources for patient care. * Expertise to deal with: End of life issues; Maternal child; Drug and Alcohol; Trauma; Ethical issues. * Expertise/Knowledge regarding documents necessary for placement and coordination of services for patients. * Able to handle on-call situations, respond to trauma, outpatient clinic needs etc.
