Community Case Manager - (Blacksburg, VA)

Overview Provides case management services for children and adolescents with developmental disabilities, serious emotional disturbance, serious mental illness, co-occurring substance use disorder or risk factors associated with those conditions who demonstrate a need for active case management services. Provides case management services in the home, school or other natural environment and coordinates services and resources needed by the consumers and their families to support their development and enable them to remain and function optimally in the community. Requirements Graduation from an accredited college or university with a Bachelors degree in Social Work or human services related position is required, with at least 1 years background in a setting working with children with behavioral health needs preferred. Demonstrated knowledge of case management/social work practices and resources available to help. Valid Virginia drivers license and safe driving history required.SDL2017
