Job Number: 172912 Department: General and Thoracic Surgery Hours: 9:00a - 5:30p In the role of a Care Manager, the Registered Nurse will support the thoracic surgery practice. This Care Manager provides support to the surgeon practices and partners with our Nurse Practitioner to facilitate a smooth referral processes, help the team achieve patient-focused outcomes, and provide longitudinal patient care throughout their surgical journey and beyond for chronic health conditions. Nursing care provided by registered nurses that function as care managers reflects the utilization of the nursing process through an integration of knowledge, skills, background, and attitudes needed to meet the needs of patients and families. Registered Nurses, physicians and other teammates collaborate to provide the most positive patient background possible.
