Case Manager - (Charleston, SC)

CASE MANAGER Responsible for coordinating services to assure that people have access to a full array of needed community services including appropriate medical; social; educational; and other needed services. The CM is responsible for identifying the person's needs and resources, coordinating services to meet those needs, and monitoring the provision of all services. Responsible for administering the Home and Community based waivers according to DHHS and DDSN regulations. CM is responsible for annual assessment and plan development, as well as accurate and appropriate billing and documentation. In addition, CM needed to serve as the point of entry for all referrals to the Disabilities Board of Charleston County, provide community resources details to potential families and consumers, and complete intake process. Applicant will have a good working understanding of community resources, be extremely organized, be able to manage competing priorities, and will be deadline oriented.
