Community Health Worker (CHW)- Poughkeepsie, NY - (Poughkeepsie, NY)

The Institute for Family Health is seeking Community Health Workers for our sites located in Poughkeepsie. RESPONSIBILITIES Under the direct supervision of the Community Health Worker Supervisor, the Community Health Worker (CHW) identifies and provides services and referrals to women across the reproductive life course with the goal of improving health and birth outcomes. The CHW will provide linkages to community resources primarily as related to health insurance, primary care and family planning. The CHW is responsible for community education and outreach, including street outreach. BACKGROUND/SKILLS Background working with community organizations as an advocate for high-need individuals. Case management tracking and EMR documentation background or the expertise to learn required. Computer proficiency and attention to detail is required. Must have a dependable vehicle and be willing to work a flexible schedule to accommodate assigned clients. EDUCATION
