RN Visit Nurse (Part Time) - (Oakdale, MN)

Essential Job Functions/Responsibilities -- Responsible to provide skilled nursing intervention and supportive care in patient---s home as assigned by the DPS to provide support and relief during nurse absence, and when RN Case Managers have high case load, or high acuity.-- -- Will participate in after hour and week end on call rotation.-- -- Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs for after-hours calls.-- Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient---s status by: Following plan of care Re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Uses health assessment data to determine nursing diagnosis.
