Hospice Chaplain - (Marinette, WI)

Position overview The Chaplain provides spiritual counseling or arranges for counseling to meet the spiritual needs of patients and families in a manner consistent with their beliefs and desires and in accordance with the patients plan of care. Key Position duties Provides direct spiritual counsel to patients/families as requested Completes initial spiritual assessment Contacts patients clergy as requested by patients and family Establishes visit plan and spiritual plan of care Provides prayer and sacraments to patients/families as requested Is alert to the theological/ethical issues which may be of concern to patients/families and teammates Assist in planning funeral services, prior to death, as requested by patients family Works with volunteer services department to place spiritual care volunteers with appropriate patients and families Serves as liaison with local religious community
