River Edge offers Community Support Services at our Bibb location. Community Support Services include services for the consumers in the community. We are the link to outside resources for our consumers. We transport our consumers to appointments, grocery store, etc. We help our consumers focus on self-sufficiency. We provide budgeting, medication, and social skills. We are seeking an SST2 Case Manager to fill 2 positions in our Community Support Services Program. Duties & Responsibilities: Provides a wide range of paraprofessional social services to patients/clients/consumers and their families, participates in treatment team planning, and performs crisis intervention, Assists in dealing with personal and social problems, Maintains medical records and documents daily group notes, Performs case management duties, and group skills training and refers clients to needed services and resources.
