Care Coordination Outreach Worker - (Hauppauge, NY)

BASIC FUNCTION To assist the Care Coordination team in the outreach and enrollment process of providing Care Coordination services to clients. SPECIFIC RESPONSIBILITIES Conducts intensive outreach efforts to persons who have been identified as Health Homes eligible by way of phone calls, written communicationand unscheduled appointments. Provides and discusses info on services to potential clients of Health Home Program and assist in the delivery of services upon enrollment. Assist in the development of the Hot Line Intake, Agency and Health Home consents. Assist in making phone calls and visits to clients, other agencies and providers in order to obtain info pertinent to the assessment, service plan development, and delivery of services. Assist and advocate in the coordination of medical/behavioral care, attend case conference meetings with other service providers and participate as needed.
