Shared Living Coordinator - (Holyoke, MA)

BHN is looking for a Shared Living Coordinator for the Holyoke/Chicopee area to provide direct services that support growth, promote independence and ensure the safety of individuals in all settings. In this role, you will: Encourage individuals' served independence, competency and decision-making. Initiate, support and ensure quality of life through empowerment, safety awareness and respect and dignity. Provide crisis intervention and emergency services as needed. Provide safe and timely transportation in own vehicle. Serve as Human Rights Officer. Assist and monitor providers in providing activities of daily living. Monitor overall health and well-being, and assists providers with all aspects of medication treatment and administration. Assist providers in maintaining, enhancing and fostering social support networks and relationships. Assist providers to plan and provide opportunities for community activities.
