Intake Specialist (Nutrition) - (Clearwater, FL)

SUMMARY Initiates, pre-qualifies and processes intake applications the OAA funded, and private pay Meals on Wheels program. Act as a liaison between referring agency professionals and potential clients. ESSENTIAL DUTIES AND Assignments include the following. Other duties may be assigned. Assists with answering, and directing, all incoming calls to the Meals on Wheels Nutrition Department. Provide alternate suggestions for underage clients who dont qualify for the Meals on Wheels program by referring from the current OAA list of services Assist outside callers/inquiries with info regarding the several Meals on Wheels program options; ie: OAA funded program, Private Pay, and any other government funded program available at the time Assist potential clients with pre-qualifying for Meals on Wheels by conducting a 701S assessment over the phone and placing the client on the MOW waitlist
