Position description: We currently have an exciting new opportunity for a Social Worker Case Management Consultant. The Case Management Consultant provides consultative services to the Case Managers, either in person or telephonically. This position consults with case management staff suggesting psychosocial interventions for complex, high-risk patients. The functions of the Case Management Consultant include collaboration with all members of the healthcare team to coordinate quality, cost-effective care and minimize fragmentation of the healthcare delivery system. The goal of this position is to assist the case managers in optimizing members? level of wellness and independence, enhance quality of life, prevent unnecessary hospitalizations or service duplication, provide continuity of care, promote cost containment, enhance quality, and improve clinical and financial outcomes.
