Family Care Liaison - (Jamaica, NY)

Little Flower Children and Family Services is seeking a Family Care Liaison for our New Life Program. This position will be based at our Jamaica Queens location. The Family Care Liaison ensures that all program and environmental requirements related to a Family Care Home are being met and maintained. Ensures that the fiscal and other provider needs and concerns are addressed and are consistent with the needs and concerns of the individuals in the home. This position is responsble for acting as a direct link between the Family Care Provider and Little Flower Children's Services New Life Program. Illustrative Examples of Work: Current understanding of OMRDD Rules and Regulations, Home and Community Based Waiver Services and the Family Care Policy Manual. Capable of understanding and working effectively with diverse personalities.
