Job Description JOB DESCRIPTION: We are looking for an experienced, compassionate, and thoughtful direct care professional to join our team as a Home Health Aide. In this role, you will provide training, mentoring, and support for each client served, targeting goals of their "Person Centered Plan", such as cooking, laundry, homemaking, shopping, safety, social recreation, personal care, behavior support, and volunteer or employment activities. You will act as an advocate for assigned clients in order to achieve their highest level of independence. This job necessitates second shift availability. PRINCIPLE ACCOUNTABILITIES 1. Understanding and complete adherence to the Person Centered Plans/Treatment Plans/Behavior Plans/Employment Plans 2. Utilization of techniques and training to provide high quality care in a safe, healthy setting. 3. Maintaining a calm, caring, motivating attitude toward client and families. 4. Respect the individual dignity, rights, and privacy of each client.
