Community Health Worker - (Howell, MI)

DESCRIPTION Serve as a Community Health Worker for the Prescription for Health (PFH) program coordinated by Livingston County Health Department (LCHD). The overall goal of this project is to increase consumption of fruits and vegetables among low-income patients with chronic diseases. The community health worker will carry out duties of the project in partnership with LCHD staff. Services will be provided no more than fifteen hours per week on days and times agreed upon and coordinated with the direct supervisor. DUTIES The Community Health Worker will be responsible for: (1) assisting in the enrollment of recruited patients in the PFH program; (2) retaining and following up on patients enrolled in the program; (3) staffing PFH table at the farmers market and food pantry; (4) providing support to patients in setting and meeting goals; (4) providing necessary assistance to enable enrolled patients to access farmers markets, food pantries
