Chaplain--Located in Battle Creek - (Kalamazoo, MI)

Hospice Care of Southwest Michigan is seeking an experienced Chaplain to provide spiritual care services to clients/families/caregivers of Hospice Care of Southwest Michigan. This position is a full time, benefited position working Monday through Friday in our Battle Creek Home Care division. As a member of our dynamic team, you will provide direct spiritual care services to clients, families, and caregivers based on the initial assessment and ongoing Plan of Care. You will provide consultation, education, and support to the Interdisciplinary Team as well as reporting on the status of spiritual care of clients, families, and caregivers during team meetings. Additional assignments include: Assures spiritual assessment of clients/families/caregivers in the Hospice Program who request spiritual care services is conducted within established time frames; Participates in the development of the individual Plan of Care to meet identified spiritual needs; Documents spiritual care services and ongoing
