Information and Referral Specialist (M642-702) - (Cambridge, MA)

Provide social services for clients of the Council on Aging, including triaging participants who request assistance; providing intake, details and referral; benefits counseling; and some case management services. Specific duties include but are not limited to the following: Triage and respond to walk-ins to the Senior Center. This includes initial screening, determination of request or presenting problem. Interview clients to assess needs. Also, screen requests for services and details from callers (including seniors, family members and other service providers). Provide info and referral; make referral to appropriate COA services and/or other provider agency. Follow up on assigned intake requests; offer necessary services and provide initial client advocacy and benefits assistance. Provide short term counseling/consulting/case management and possible home visits when assigned by the Director of Client Services.
