Provide outreach and case management services as a part of the Tumaini Center outreach team. The expertise to effectively engage and encourage the motivation of consumers from the homeless population. Connect consumers to resources, ensure appropriate treatment, evaluate housing situation and general quality of life. Job includes working on various shifts and on-call shifts.
