CCCI has a Full time Transition Coordinator position available starting in our Wethersfield, CT office and moving to our Bristol, CT office to serve the North Central region of CT. The principal role of the Transition Coordinator is to provide details and outreach support for the Money Follows the Person program, facilitate the transition of eligible individuals, and to perform all related data entry and reporting.
