Outreach Worker, Family & Community Services - (Norristown, PA)

Duties and Responsibilities : Provide family-centered, community-based services to pregnant women referred for service due to substance use, including teaching life skills, advocacy, parenting education, info, referral and counseling. Provide counseling services that are family-centered with a systemic approach and are focused on decrease or cessation of substance use & on D&A recovery. Assess risk of child abuse and neglect and provide appropriate interventions with families. Assess client and family functioning, plan goals and identify resources. Maintain and complete accurate paperwork including assessments, plans, progress notes and reports within required time frames. Involve client in planning outcomes. Work with clinic in planning and coordinating service. Demonstrate a current expertise of available community resources by providing appropriate info and referrals. Actively network in the community in order to build on local assets to strengthen families
