Service Navigator â?? Bilingual (English/Spanish) - (Rochester, NY)

PRIMARY FUNCTIONS : Provide support to the Care Managers (CM), Engagement Services staff, SC (Service Coordinators), CC (Care Coordinators) and MSC (Medicaid Service Coordinators) within the Care Coordination Department. SPECIFIC DUTIES: 1. Assist CMs, SCs, CCs and MSCs with client appointments and accessing needed services as identified on clients care plans/service plans/ISPs. 2. Provide support to clients in completing applications and paperwork for and management of benefits, financial, housing, or other related areas as needed. 3. Assist in Outreach and Engagement (O&E) activities such as distributing program brochures, locating eligible clients to build caseload, maintaining contact with potential referral sources and completing intake paperwork as applicable. 4. Provide assistance to find clients lost to care" and maintain ongoing contact with enrolled clients, as requested. 5. Maintain accurate and verifiable documentation of all services rendered.
