Recovery Specialist - PITCHESS Jail Health Services - (Los Angeles, CA)

JOB OVERVIEW: Assist with admission into SUD services, transitioning from 1 level of care (LOC) to another, navigating the mental health, physical health and social service systems and carry a case load as assigned by program management. Communicate with a network of providers as participants move between LOCs and into post- discharge recovery services to support successful transitions back into the community. Provide individual and group counseling/case management, regular assessments, and crisis intervention in accordance with the approved Evidence Based Practices (EBP). Provide evaluation; plan development; placement services; assistance in daily living; linkage and consultation to Substance Use disordered clients. Provide referrals to adjunct services in relapse prevention, independent living skills training. Provide outpatient follow up services upon discharge. Provide comprehensive substance abuse services to clients while housed in within the jail to determine level of treatment compliance.
