Family Support Specialist-Partnership for Children-Polson - (Polson, MT)

The Family Support Specialist works as a member of a team supporting a youth in their family, group home or other living situation. The Family Support Specialist works 1 on one with the youth in coordination with a treatment plan overseen by the Family Care Program Manager or Treatment Coordinator. The specialist primarily provides and meets the standard for ?community based psychiatric and rehabilitative services? (CBPRS) or similar services required by contract. Nature and Scope of this Position: ? Position is supervised by the Family Care Program Manager. ? Position works closely with Therapists and Treatment Coordinators. ? The roles of the Position are defined in the youth's individual treatment plan. Examples of Tasks: ? Responsibilities are directed by a Treatment Coordinator or Therapist to provide supplemental in-home treatment and support to children and families involved in the Therapeutic Foster Care Program, Home Support Services or a consumer with the Mental Health Center.
