Program Officer, Patient Care Program - (Palo Alto, CA)

The Patient Care Program is seeking a Program Officer to provide programmatic, strategic and implementation support to the Patient Care Program's portfolio focused on care for people with serious illness. The Program Officer will collaborate with other members of the team and engage in strategy development, grant development, grants management, internal and external communications and relationship building, and will contribute to the overall effectiveness of the Program. Critical responsibilities Specific assignments include: Strategy Build solid knowledge of the field and key emerging trends in community-based serious illness care and, in particular, healthcare workforce development. Summarize literature and identify implications for the Program. Identify, size, and assess potential opportunity areas. Identify and engage subject matter experts and other stakeholders. Set goals at strategy and grant-level and measure progress and achievements against those goals.
