Care Manager - (Lima, OH)

PROFESSIONAL Area Agency on Aging three is advertising for the following position: CARE MANAGER Assignments include but not limited to develops/coordinates/manages care plan for services for all ages. Advocates for services and benefits, conduct family/service provider case conferences, arrange for service delivery and reassess needs and services for care. Requirements: RN or LSW required, 1yrs prior work in community health care, general knowledge of Medicare/Medicaid and community resources needed. Outstanding communication abilities, expertise to multi task, meet deadlines, be organized and detail oriented in a fast pace environment and computer experience is a must.
