Care Navigator, Southeast Washington - (Richland, WA)

The Care Navigator educates the community about Alzheimer's disease and related dementias and about the Alzheimer's Association and Southeast Washington Aging and Long-term Care (SE WA ALTC); connects community residents to community and Chapter programs and services; coordinates and supports service delivery in the assigned community; and cultivates and sustains strategic partnerships and alliances with community-based businesses and nonprofits. This position has three general categories of responsibilities: Directly provides and/or assists in the delivery of and volunteer recruitment and training for outreach efforts, educational presentations, early stage programming, support groups, and events in counties served by SE WA ALTC, in conjunction with SE WA ALTC and Alzheimer's Association staff. Counties include Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, and Asotin.
