Home Visitor (Home Based Option) POSITION DESCRIPTION: Provides educational and support services to low income infants and toddlers and their families during weekly home visits and socialization activities in Early/Head Start centers. SPECIFIC RESPONSIBILITIES: Key functions: Acts as a liaison and advocate between community resources and Early Head Start families. Collaborates with families to establish family development goals, and document this collaborative process in family partnership agreements. Completes documentation and maintains confidential records of home visits, socialization experiences, and other relevant activities. Conducts screening and ongoing assessment of young children's development. Conducts weekly home visits. Educates and monitors parents with respect to children's health, nutritional, dental, mental health, and social needs. Promotes parental involvement in curriculum planning, program governance, parent training, and the community.
