Job DescriptionWe are seeking a Home Health Aide - Chore Provider - Home Maker - Caregiver to become a part of our team! You will provide routine Activities of Daily Living (ADL's) along with performing chores according to the provided Time & Task Sheet all at the patient's home.Responsibilities:Provide routine individualized care and ADL's to the elderly, convalescents, or persons with disabilitiesMonitor and report changes in patient health status to the immediate SupervisorProvide personal care, such as bathing, dressing, grooming and in special cases, incontinence carePerform chores according to the Time & Task Sheet such as light housekeeping, grocery shopping, errands, meal prep, laundry and medication remindersProvide companionship and listen to the patients, alerting the Supervisor of any concerning or major incidencesQualifications:Background in hospice, home health
