Parent Educator/Home Visitation Worker - (East Hartford, CT)

Title : Parent Educator / Home Visitation Worker Reports to : Manager Department: PAT/Parents as Teachers Status : Non-Exempt Job summary : The Home Visitation Worker is responsible for conducting parent education home visits under the Parent As Teachers Program. This position mandates outreach, recruitment, and comprehensive risk assessments of high risk parents who may be eligible for maternal, infant, and early childhood home intervention services. Deliverables also include implementation of ongoing periodic screenings and evaluations of parents and children during weekly home visitation sessions, individual and group level parenting education and interventions, and concurrent documentation of contacts, data and progress reports. The Home Visitation Worker provides and coordinates case management and support services to participating families. Primary Accountabilities and Tasks:
