Job DescriptionRegistered Nurse (RN) Care Manager - Janesville My Choice Family Care is a leading managed care organization serving 9 Wisconsin counties. We are seeking a licensed Registered Nurse to join our Care Management Team at the Janesville office. This position is responsible for providing ongoing nursing care management through regular in-person and phone contact with Family Care members, developing and maintaining care plans that assist members to live and function as independently as possible. The RN Care Manager collaborates with the Care Manager to assure the health and safety of those enrolled in the Family Care program. Our preferred applicant will demonstrate solid critical thinking skills and a passion for excellence in customer service. Candidates must be a licensed RN by the State of Wisconsin with a min of 1 yr background working with individuals who have developmental, intellectual, physical disabilities, or frail elders. Bachelor degree in Nursing preferred.
