PACT Team Leader Job - (Seattle, WA)

JOB DEFINITION Administer, and direct the PACT team, a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified participants with severe and persistent mental illnesses. Supervises and evaluates the trans-disciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program participants. Functions as a practicing clinician on the team. This position has training in 1 of the mental health professions (e.g.,nursing, social work, rehabilitation counseling, or psychology) and clinical skills and background to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to participants with severe and persistent mental illnesses and co-occurring substance use disorders under the clinical supervision of the team leader, program manager and the psychiatrist.
