Permanency Placement Services Clinician - (New Britain, CT)

Provide an array of services for assigned case(s) in the home and community to support reunification and/or permanency support for DCF-involved children, youth and families. Services may include providing parent education, developing family resources, permanency readiness, and/or pre-and post-reunification support. C omplete case planning, thorough documentation of all case service activities, monthly case updates, and assessments as required. This job necessitates availability to deliver services on Saturday and/or Sunday. May require extended driving and/or transportation of clients.
