Registered Nurse - Anticoagulation - Full Time - (Corning, NY)

Main Function Performs anticoagulation studies in a highly structured environment by collecting specimens, taking vital signs, and patient history. Assessment of anticoagulation report and medication adjustment based on standardardized protocols and algorithms. Collaboration with medical director on new patient referrals, risk assessment scoring, and bridging therapy qualifications. Documents, reviews and enters all patient data in the electronic medical record including medication review. Provides ongoing patient education and counseling. Generates reports to providers. Assists patients with medication management. Position mandates proficiency in mathematics and the expertise to convert milligrams per week into number of pills per day. Strong pharmaceutical knowledge. Education RN , PA and NY dual license preferred. Background
