Open Date 6/27/2017 Division Reading Hospital Description The Ambulatory Clinical Quality Coordinator Level III-THMG shall work together with and under the direction of the Director, Ambulatory Clinical Quality, THMG to improve performance by creating, implementing and evaluating ambulatory clinical quality programs. This position's overall purpose is to support the quality-related aspects within ambulatory practice settings for reducing care fragmentation while optimizing patients' individual background of care, improving health of populations, reducing costs of care for populations and enhancing care team satisfaction (Institute of Healthcare Improvement Quadruple Aim). This role collaborates with the Senior Director, Ambulatory Nursing, Director, Patient Centered Medical Home and the Director, Ambulatory Medical Record Quality to monitor and improve clinical processes and documentation in support of the quality programs for The Tower Health Medical Group.
