Community Treatment Team Program Assistant - (Pittsburgh, PA)

Additional Job summary Critical functions of this Position: * Facilitates team communication by acting as a 'hub' of case management related knowledge. * Sees to consumer needs when consumers present themselves at the CTT office. * Observes daily medications compliance of consumers at CTT office. * Distributes weekly spending checks to clients who receive Mercy Behavioral Health Representative Payee Services. * Enter data into the Allegheny County Health Choices (ACHI) database. * Triage crisis calls. * Answer and route incoming calls to the CTT team. * Exercise clinical judgment in initiating services and scheduling appointments for consumers. * Exchange details with involved parties as required. * Coordinate and schedule routine, urgent and well visits with providers for individuals receiving services. * Assist team leader with coordination and organization of daily team operations. * Provide daily support to CTT staff, team leaders and individuals receiving services.
