RN Maternal Child Health Home Visitor - (Wilmington, DE)

The goals of Healthy Families America Program through Delaware HOPE are to improve birth outcomes, reduce rates of child abuse, strengthen families, enhance child health and developmental outcomes, and promote family stability and economic self-sufficiency to parents with substance use disorders Provide intensive home visiting to low income pregnant women and their children up to 3 years of age. Nurses carry a case load of ten high risk mothers and their children. -- Following evidence-based curriculum, ---Partners for a Healthy Baby---,--the nurse will--provide support to family in--areas such as--family development, maternal and family health, preparing and caring for an infant or toddler, and infant and child development. The home visitor will be provided with weekly reflective supervision sessions to promote continued growth with the program.
