Job overview: RN CASE MANAGER - HOPICE CARE - HOME HEALTH AND COMMUNITY LIVING On-call required every three weeks Main territory - Wheeling Secondary Territory: Prospect Heights The registered nurse/case manager assesses patients and their needs. They manage the patient's case and make appropriate suggestions regarding pain and symptom management in order to ensure comfort and quality of life. The registered nurse/case manager educates and supports patients and families regarding expectations during the end-of-life process and is responsible for the overall coordination of patient's care. This RN will cover the area surrounding Schaumburg and Wheeling Essential Duties and Assignments include the following: Provides patient care visits for admission, assessment and pain and symptom management and support during the dying process. Educates current and prospective patients and families regarding hospice and the end of life. Documents patient visits and phone calls in an accurate and timely manner.
