Director of social work (mds 3.0 experience required) - (Archdale, NC)

There is an immediate team opening available for a Director of Social Work with a prominent senior services center, The GrayBrier Nursing & Rehabilitation Center.We are a family-owned and operated Nursing Facility in the Archdale/Trinity area. Compensation is commensurate based on background. Benefits include: PAID-TIME-OFF, health insurance, dental insurance, and vision insurance. This is a full time, hourly position. Position Functions Include Assign and/or monitor to assure appropriate sections of MDS assessments are completed. Complete Medicaid applications, FL2s, PASSR, etc. as required. Ensure resident's psychosocial, emotional, and mental needs are met as described in the program. Work together with the Social Work Assistant and other departments within the facility. Coordinate room changes and discharge planning for facility residents. Attend facility meetings with the interdisciplinary team, residents, vendors, and family members.
