Team Assistant - (Medford, OR)

Providence is calling a Team Assistant (1.0 FTE, Days) to Providence Medford Medical Center in Medford, OR. In this position, you will: Support Providence Medford Home Health & Hospice Triage, Intake, Utilization Review and scheduling. Perform specified demographic and payer source data entry of referral/intake details into Home Care's software system(s). Make calls to physician offices, hospital or other referral source to obtain additional patient demographic & payer source info in order to verify insurance eligibility and benefits for referral as requested. Communicate new referral details to physician, Home Health and Hospice vendors or other entities via fax/phone/computer as requested. Prepare (IDT) & PCC agendas Perform staff/patient scheduling. Report statistical data. Perform other office support functions as requested.
