Job summary Will assist elderly clients in developing and maintaining max independent living. Will be responsible for the assessment and development of service plans and goals. Job necessitates home visits to elders, knowledge of and extensive contact with community social service agencies, precise record keeping and written correspondence expertise. Social work background and knowledge of social services preferred. Sensitivity to needs of elders essential. Min Requirements : BA or BS in Social Work, Human Services or related position, background in case management with geriatric population. LSW a plus.
