Job summary ESSENTIAL DUTIES & RESPONSIBILITIES: Conduct departmental screening and intake for program eligibility Conduct Global Appraisal of Individual Needs (GAIN-Q) assessment to identify clients’ treatment planning needs Develop and monitor treatment recommendations and individual case management plans for each client Provide referrals and linkages to outside service providers, as needed Provide individual substance abuse counseling Collaborate with our partnering residential treatment providers Develop and implement culturally appropriate substance abuse treatment plans and aftercare services Facilitate culturally sensitive recovery groups, relapse prevention, aftercare, and substance abuse education workshops Collaborate with other members of the departmental treatment team, including therapists, psychologists, psychiatrists, social workers
