Mental health care coordinator - (Los Angeles, CA)

Position description Essential Duties, Functions & Responsibilities: Serve as the initial and primary contact with clients in 7 Generations and is responsible for coordinating the services for each client assigned. Provide in –home, field-based care coordination services and/or office-based services for clients with a range of psychological, emotional and behavioral problems. Needs a valid driver’s license and vehicle, as the majority of visits will be field based, so there can be a lot of driving. Identify needed resources for child and family; provide referrals and direct linkages to resources and services. Develop and maintain community-based referrals to programs for children, youth, adults, and their families. Interface with community agents (eg schools, Department of Child and Family Services, Probation Department., etc.) on behalf of clients. Outreach to the community to promote awareness of the programs available at UAII on a regular basis.
