Physical Therapist (PRN) - (Nashville, IN)

LifeSpan Therapy, a CarDon & Associates partner, is now seeking a PHYSICAL THERAPIST. The Physical Therapist provides compassionate and quality rehabilitation services to residents under the directive of a physician and in collaboration with the Interdisciplinary Care Plan Team. The PT performs therapy or assigns resident to a Physical Therapy Assistant (PTA), communicates sufficient info about the treatment plan to nursing and other personnel to assure a team approach. Documents therapy services and writes progress notes in accordance with Medicare Regulations.
