LCSW / Behavioral Healthcare Coordinator Wellsboro, PA sj THE LOCATION: Wellsboro, Mansfield, Blossburg, Lawrenceville, Elkland and Westfield, Pennsylvania Permanent / full-time / employed Ready for a change? We have an exciting opportunity available for a LCSW to join our team in Wellsboro, Pennsylvania. You will be functioning as a Behavioral Health Care Coordinator and core member of a collaborative care team which includes the patient?s medical provider, a psychiatric consultant, a larger primary care team, a behavioral health team and support services. You will be responsible for supporting and coordinating the mental and physical health care of patients and you will have an assigned patient caseload. Services will be delivered at the main office in Wellsboro, as well as five other clinics in Mansfield, Blossburg, Lawrenceville, Elkland and Westfield. You will be providing services to a varied patient mix including children and adults of all ages on an individual, family and group basis.
