Skills Coach Multi-lingual - Otis R Bowen Center for Human ServicesJob Description The skills coach is responsible for providing case management and skill building services to an assigned caseload. As a Skills Coach, you will provide services at Bowen Center locations, in the community, and in client's homes. Your caseload may consist of child/adolescent or adult population depending on the location and the need. Duties of this role may include: Facilitate clients' learning and development of new skills, improving functionality and independence Ensure clients are linked with other needed healthcare providers Ensure clients are addressing the goals that are important to them and/or their referral source Participate in medical appointments with the organization's medical staff Ensure clients are making progress and are discharged within an appropriate timeframe Transport clients as needed and appropriate to their care Job RequirementsRequirements: BA/BS required 1-two years of professional background preferred
