Case manager- homeless youth - (Riverdale, IL)

Job overview The purpose of the Case Manager is to provide day to day support, encouragement and referrals to clients to improve their functioning and improve the client’s outcomes and satisfaction. ESSENTIAL DUTIES AND RESPONIBILITES include the following: Screen, determine eligibility/appropriateness and enroll eligible and appropriate clients consistent with program plan. Complete all funder and/or agency required assessments, service plans and other forms (CIS, Ansell-Casey, progress notes and other required paperwork). Execute routes as assigned by the supervisor in either an agency vehicle or one’s personal vehicle. Visits may be in office, home, school or worksite as program qualifications dictate. Ensure that client is served in a Trauma Informed Care approach Liaison and networking with law enforcement, probation, employers, family members, schools, health providers and any other providers involved in the client’s service planning.
