Residential clinical coordinator - (Elgin, IL)

Position details Residential Clinical Coordinator is responsible for the clinical case management of assigned caseload. Ensures that all aspects of community living comply with all applicable Federal, State, and Accreditation standards. Responsible for developing and implementing individual programs to maximize the client’s independent community living skills and inclusion in their community. Provides documentation on assigned clients in accordance with DHS, CARF, or DPH standards, as applicable. Advocate for and maintain rights and privacy of clients that are served. Accountable for providing to both clients and staff members a positive, professional and safe environment to live, learn, and work. Responsible for a permanent caseload of up to 25, and additional cases as assigned based on the needs of the program. Supervise support staff as assigned. Provide staff training for client strategies and/or programs and assist with new hire training as assigned.
