Case Manager RN Location: Fort Myers, FL Department: Care Management Categories: Nursing Type: Full time Shift: 8:00 AM-4:30 PM Schedule: Days Description: The Case Manager has clinical ability with knowledge of medically appropriate criteria, and applies principles of utilization and quality management, discharge planning and the management of clinical/financial resources as a facilitator and active participant of the multidisciplinary patient care team. The Case Manager predicts, facilitates and evaluates the transition of patients across the care continuum and links people to systems that provide them with resources, services and opportunities to achieve and maintain max functioning.
