Meet regularly with their clients to help them gain access to medical social, educational and psychiatric services Create individualized care plans Work with direct care providers Follow up with their clients to make sure they are following their individual plan As a case manager at Valley, you have an opportunity to work in 1 of the variety of treatment programs we offer our clients. Our case managers work in residential and outpatient programs that cover homelessness, addiction recovery, and behavioral health disorders, with adults and with children.
